TOPDOG DOG WALKING — LIABILITY WAIVER & RELEASE OF RESPONSIBILITY
Assumption of Risk. I, the undersigned pet owner, acknowledge that dog walking involves inherent risks including, but not limited to, physical injury to my pet(s) from environmental hazards, interactions with other animals or people, or unforeseen circumstances. I voluntarily accept these risks on behalf of my pet(s).
Release of Liability for Injury or Escape. I hereby release and hold harmless TopDog, its walker(s), employees, and agents from any and all claims, demands, damages, or expenses arising from injury, illness, death, or escape of my pet(s) during services, except in cases of gross negligence or intentional misconduct.
Vaccination & Health Confirmation. I confirm that my pet(s) are current on all required vaccinations (including rabies), are in good health, and have not been exposed to any contagious disease within the past 30 days. I understand that my pet(s) may be refused service if they appear ill or show signs of contagious illness.
Property Damage. I acknowledge that TopDog is not responsible for damage to my property resulting from normal wear and use during services. I agree to maintain adequate homeowner's or renter's insurance for any property-related claims.
Non-Aggression Acknowledgment. I confirm that my pet(s) have not bitten, attacked, or shown unprovoked aggression toward any person or animal within the past 12 months. If my pet(s) bite or injure any person or animal during services, I accept full legal and financial responsibility for any resulting injuries, damages, or legal costs.
Emergency Medical Authorization. In the event of a medical emergency, I authorize TopDog to seek veterinary care for my pet(s) at my expense. TopDog will make reasonable efforts to contact me first, but may proceed if I cannot be reached and care is urgent.
Governing Law. This agreement shall be governed by the laws of the State of Washington. If any provision is found unenforceable, the remaining provisions remain in full effect.
